2)   please download and fill out the iPlay enrollment form.
3)   Submit completed enrollment form to m.i.l.k. Causeway Bay either in person, by mail, thru fax (28569803) or email (admin@imilk.co).
4)   We’ll contact you as soon as possible to finalize the registration.
